Receive Continuity of Care Documents (CCD’s) to your Electronic Medical Record

In an effort to provide optimal patient care, the HCA Health Information Exchange department has developed two new solutions to better share information between the hospital and our referring providers.

Electronic Delivery of Continuity of Care to PCPs

The first solution is a single direction Continuity of Care Document that is sent from West Valley Medical Center to the patient’s primary care provider once the patient is discharged. Beginning in June, CCDs were automatically sent to PCP’s upon patient discharge if the following two conditions were met:

  1. The patient must communicate their primary care provider to the registration team at the hospital prior to patient discharge.
  2. The patient’s primary care provider must have a Direct Address that is published with a health Information Service Provider (HISP).

If you are a primary care provider and are interested in receiving CCDs for your patients after discharge, contact your HISP provider and ask them to publish your direct address.

Community Exchange Partner Program

The second solution is the Community Exchange Partner (CEP) program, which helps to cultivate valuable relationships allowing for the electronic exchange of encounter-based information to support better patient care. The CEP exchange is a bi-directional exchange of CCDs, working in the following manner:

  • The Community Exchange Partner sends West Valley Medical Center a CCD prior to the patient being admitted. The patient is treated at and discharged from the hospital and a “close the loop” CCD is returned to the partner automatically at patient discharge.

The Community Exchange Partner program can be implemented with any practice that has obtained an EMR system at its own cost that is meaningful use certified. It is part of the capabilities inherent in EMRs.
